The version you get is not the version they live.
Telling you a problem early and in full costs a manager something. Softening it costs them nothing. Both of them are behaving sensibly, and so is the person above them.
Do that across five layers and the softening adds up in one direction. What lands on your desk is tidy, current and wrong in the places that decide.
The judgement is sound. The picture is not.
This is not about dishonesty. It is about what the structure rewards. And it is the reason capable leaders make decisions that look inexplicable a year later.
What gets dropped on the way up
The bad number, while it is still small
A manager who brings you a bad number early owns it. A manager who waits until it is too big to hide is handing over a crisis, and by then it belongs to the business rather than to them. Waiting is the safer choice for the person doing the waiting, every time.
The objection that was raised and lost
It gets settled before the meeting, so the minutes record agreement. The record is accurate about the vote and silent about the argument.
The clash between two of your own priorities
Take either priority on its own and it is sensible. Put them together and something has to give. Saying so out loud means saying which one loses, and that is a decision only you can make, so the level below you leaves it alone and works around it instead.
Three ways to hear it unfiltered.
You cannot fix this by asking harder questions down the same line. The line is what is doing the filtering. What works is a route that does not run through it.
Take the name off it
People say what they know when it cannot be traced back to them. Not as a promise you make, but as a condition you build. You get the transcript, not the source.
Go round the line
Read the evidence directly instead of a summary of it. Board packs, exit interviews and minutes tell you what happened. A report tells you what somebody decided you should know.
Take the cost out of speaking up
Raising something has to cost the person raising it nothing. That is what keeps it working after the first uncomfortable thing gets said out loud.
Ask a business what is wrong and it will tell you what it has agreed to say is wrong.
